Federal and State agencies are increasing pressure on MCOs to track and report on social determinants of health (SDOH) and health equity metrics.
But most MCOs lack tools to integrate internal data (claims, enrollments) with external sources (census, crime, education, etc.)—resulting in fragmented views of member health and missed opportunities for proactive care.
The Solution
As Pelagorn Health’s product partner, we led the end-to-end MVP delivery—from concept and UX design to launch on a scalable tech stack—using our 4-Step MVP Framework. The platform combines internal MCO data with external SDOH insights and leverages OpenAI to generate actionable care strategies.
The platform enables:
